Program Overview – Community Care, Seniors Active Living Centres & OutreachThe program is to provide enhanced support services to seniors pre and post discharge from hospital to home. The target population is older adults (55+) being discharged from ETHP hospitals into the local community. The EHSP Coordinator is responsible for program administration; direct connection to local and partner agencies using the funding to support older adults who are returning home; tracking program spending and resources used.

Why You’ll Want to Work With Us- Develop, implement and evaluate the EHSP in collaboration with the Seniors Community Programs Director.
- Serve as a point of contact information for the EHSP regarding service inquiries, and general program information.
- Promote and conduct outreach for EHSP.
- Act as a representative on associated committees and rounding tables.
- Oversee program expenditures by monitoring budgets, processing invoices in SAP Concur, and maintaining accurate documentation to track requests and spending with internal and external partners.
- Complete and maintain all records and forms regarding program/service enrollment, using an internal tracking system.
- Process requests and assess eligibility documentation in accordance with program eligibility criteria.
- Prepare program reports, statistics, and documentation for internal use and external audits.
- In consultation with the Director, respond to community needs, ensure existing and new service offerings meet these needs.
- Ensure data confidentiality and compliance with privacy regulations.
- Coordinate and oversee EHSP volunteers, as needed.
- Support quality assurance activities, including satisfaction surveys and other feedback mechanisms.
- Respond promptly and professionally to all concerns related to the EHSP, escalating issues to the appropriate persons when necessary.
- Ensure the anti-racism objectives and community development principles of the organization are achieved within the program
- Participate in regular supervisory sessions with the Director.
- Responsible for upgrading training and education as directed by WoodGreen Community Centre to fulfill the responsibilities of the job.
- Familiar with all of WoodGreen’s emergency procedures and staff telephone numbers.
- Perform other related duties as required from time to time.
What You Bring to the Team
- Bachelor’s degree in social work or related major
What Will Set You Apart
- Good understanding and knowledge of seniors’ issues.
- Good understanding of social and wellness related to aging.
- Knowledge and demonstrated skills in program development and management.
- Documentation, basic accounting, and MS Office suite proficiency.
- Able to work independently with minimal supervision.
- Good multi-tasking and strong research skills.
- Knowledge of community resources for older adults; interest and experience in event coordination.
- Due to WoodGreen’s multi-site locations we reserve the right to relocate the incumbent to another site, in the same position performing the same duties, as may be dictated by program needs. Reasonable notice of such site relocation will be given.
